 METHOD OF PAYMENT We are pleased to inform you that we are providers for Anthem Blue Cross/Blue Shield and Delta Dental. If your coverage is with another insurance company, we will be happy to file your claim for you, but we do ask that you pay your deductible and co-pay at the time of surgery. It is our office policy that all fees be paid on the day of surgery unless arrangements are made in advance.
